We are very happy to host your class trip to MLBS. For liability and
"animal care and use" reasons we require a few things in advance of
your arrival at MLBS. Thanks very much for your attention to these important
requirements.
1) Make sure the MLBS office knows about, and has approved, your
trip well in advance. Please send us the name and dates of
your course and a list of all participants
2) All participants (students and instructors) must complete the on-line liability
waiver. Click
here or use the side bar button to complete the waiver.
3) Instructor
must supply copies of any applicable permits. Photocopies or faxes are
fine.
4) Courses using vertebrate animals only: Instructor must
verify that all course activity will fall within the procedures described
in the MLBS Omnibus Course Animal Use Protocol. We will ask that
you review appropriate protocol text.
5) Courses using vertebrate animals only: Instructor must
supply a brief statement of training / experience (as you might include
in a course animal use protocol). Email to enagy@virginia.edu
preferred.
